This form can be used for cases with up to 4 attorneys, and up to 5 people who need to be notified of the LPA.
You must not Cut or Paste any pages within this form. Continuation pages may be Copied and Pasted to the end (only) of the form.
You can 'Print Relevant Pages to Single PDF File' button on page 1 to print all completed sections as a single printstream - useful for creating a single PDF file.
This option is intended to allow users to output a single PDF file via their PDF printer driver, which must be set as the OyezForms default printer. NB: you cannot target Oyez's bundled 'OyezLegalForms PDF' printer driver for this task as license restrictions limit its use to via the application FILE menu.
You can target your PDF printer as OyezForms' default printer via the TOOLS>OPTIONS>PRINT dialogue. Oyez recommend CutePDF, a freely availble PDF printer driver.
